Staffs Partnership this is of no Suprise

Hi folks

It has been some time since I last wrote to update yo.

I have just spent 2 hours going through the latest CQC report regarding this trust.

All my worst fears have been confirmed

The trust are not learning from mistakes and have no grasp of the values it is attempting to peddle.

There is insufficient  training and evaluation of staff it is of no surprise to me that this trusts social workers were going unchecked when it came to professional conduct

There can be only one answer to this CQC report

Special Measures

God help the people of Staffordshire and these unvalued staff

Report Of Social Care Records Gained Under Freedom of Information

Christmas is nearly here and I have been quite busy as usual, In my last post I advised you all of the receipt of the long awaited report which was being withheld by the Staffordshire & Stoke on Trent NHS Trust.

The report makes for heavy reading and I advise the reader to view it and make your own conclusions.

As always you the reader do not know what occurred in relation to my late granddads care, however I can assure all that you are about to read is defamatory and fictitious and has been proven as such.

Records that were recorded by many individual Social Workers charged with my Late Granddads Safeguarding are both fiction and lies with many points of concern left without validation or raised as safeguarding concerns.

The report covers 4 main Vulnerable adult referrals and action/omissions over a period from 19/09/2006 to 2015.

This report highlights total lack of professional conduct in areas of Adult Social Care.

The Audit report referred to in this report Highlighted concerns of financial mismanagement not just isolated to my granddads care.

The Social worker concerned has apparently been disciplined for not keeping appropriate records or receipts, to date there are monies still unaccounted for in the estate of the late Ronald Bourne.

This Individual is still actively A social worker working with Vulnerable adults these many years on and has not been reported to the professional body charged with social work by the employer.

My advice still remains the same, here in this area it is impossible to trust any Social Worker.

As Supposed evidence will be created and vexatious defamatory remarks will be made about the client there family and carers.

Your loved one will be cajoled and be lead in conversations irrespective of there mental state at the time.

If you are two males looking after A Vulnerable adult you will be highlighted as either a couple or two busy in bedroom to care,  there is no low that these vile individuals will go to

They will report that you have abused them by leaving them on the floor and quote you directly with deceitfull comments.

All of this will take place behind your back and without your knowledge to the detriment of the Vulnerable Adult.



When Dealing with Adult Social Care you must :

Record all meetings As these will be misreported

Keep copies of all care plans and notes as Invented copies fraudulently created will be relied upon to destroy your credibility.

Never dare to complain when elderly care and carer support is poor as I did in 2006 as A Witch Hunt will ensue.

Do not bother with your local Mp if the main proliferate of the witch hunt is an Mp herself.

Never accept that a past CEO of a county Council is going to carry out any form of Independent Investigation based on fact when being paid for and checked over by solicitors at Staffordshire County Council.

The Legal position from the family solicitor is that trying to get Transparency or Change out of this trust or the County Council will be protracted and very drawn out costing many thousands of pounds.

Finally all I ask the reader is why have I been willing to share this document ?

Because I want honest decent reporting and investigation when it comes to Vulnerable Adults

Is that to much to ask for ?

And now the sad news is that my fight for justice for myself and granddad is over and I can never replace the years that he was forced into that care home.

Sadly I will never be able to hold this Trust or the County Council to account for these vile acts.

My own health has suffered these many years and it is time for me to move on.

However thanks to all of what I have witnessed I am and always will be a Activist for the rights of the elderly and there carers.

Transparency At Stoke On Trent Partnership

The Trust have today complied with my FOI Request for A report requested by the CEO of the local NHS Trust with regard to the conduct of Social care Professionals.

The Report must now be digested and forwarded to the family solicitor for his further advice.

In my uneducated view the document shows a Witch Hunt with no regard for Adult Safeguarding and A complete disregard to my late Grandfathers Rights when it came to Mental Capacity.

The document further goes on to suggest that Social Workers were leading my late grandfather in the manner in which investigations were apparently being conducted.

Further findings report that a specific individual has been disciplined, however the individuals serious failings Have not been reported to the HCPC who are charged with upholding public accountability.

On the positive side I can now see for myself how one individual social worker can cause the complete destruction of family life and cause a raft of vexatious Social Workers to not follow evidence and embark upon nothing more than a Witch Hunt.

My fight can not rest the Trust have actively protected the County Council, Itself and its rouge Social Workers.

The 19 pages of this report are a very disappointing review of Adult Social Care and Safeguarding and those responsible must be held to account, There is no place when charged with the safety of vulnerable adults for unprofessional,deceitful and blatant lairs.

I will update you all shortly as the disclosure needs to be checked against the ICO due to possible disclosure issues.

Public Decision By The ICO regarding Staffordshire and Stoke On Trent Partnership Trust

I might not update as much as I usually do however my focus is still set solid on
gaining some Transparency regarding the Stoke on Trent Partnership Trust.

Today my family are one step closer to some answers.

The Information Commissioner has found that my request to the trust was not vexatious and does not apply to my Request to the report which is being witheld by the Trust.

What I have found to have been consistent over the last few years is the underhand way in which organisations will go out of there way to discredit individuals integrity.

The trust provided the ICO with many statements regarding myself being nothing more than a vexatious complainant attempting to discredit individual social workers with specific reference to my use of this website and also Twitter.

A Further argument was made by the Trust was that my initial complaint with the County Council and then the Trust was Independently Investigated so therefore all issues were resolved lol.

My personal view is that the so called Independent Investigation was nothing more than a cover up to protect many individuals including the county council and the local trust.

Notes for the Day.

Being persistent against ignorance is not being Vexatious.

Telling the truth about Social Workers and there conduct when failings are being covered up is nothing more than Public Whistle Blowing by concerned members of the public.

Culture Change at Stoke on Trent NHS Partnership & Not fully Investigating Failure

Just to keep you all updated, I am awaiting the Information Commissioners views on my request which  I hope should finally expose the extent of the damage to my own personal reputation and the appalling failures that the Stoke on Trent Partnership trust have allowed to take place

These involve the use of fraudulent home care records and deceit and lies by many members of its own staff and this organisations miserable attempts to protect its own reputation.

Up to the date of this blog no action has been taken against Mrs A Trubshaw a social worker employed by the trust for directly deceiving the Department for Work and pensions.

Also to date no criminal charge or investigation regarding the direct Threatening conduct by Mrs Bethan Lally Social Worker.

Further to date no action or investigation is apparent into multiple NHS and Social Care staff utilising a fraudulent set of care records to discriminate directly against both myself, late grandfather and extended friends.


So with the lack of transparency accountability and the Trusts attempts to suppress the extent of there own failings my concern became that if this trust were so intent to protect itself from genuine public complainants what sort of arrangement was in place for there own staff.

We are informed that employees have an Ambassador for Culture Change so I reviewed the reasons for this post which is directly accountable to the CEO Mr Stuart Poynor.

Mrs Helene Donnelly is the ambassador for Culture Change at the Trust however the posts remit appears to not include how the Trust engages with service users or none employees.


On the trusts website the new Ambassador for culture change says these are her aims.

  • How supported do staff feel?
  • Are staff confident that they know how to raise concerns?
  • Have staff raised concerns in the past? What has been the response?
  • Do staff feel listened to?
  • What helps and hinders staff from doing their jobs well?
  • What do patients/ service users and relatives feel? 

The above is very commendable and appears to be the trust heading in the right direction, So with one of her aims being about how service users and relatives feel I felt it would only be right to send her an email and ask for her views.

There has been no reply to my communication so I am left with the only opinion that the contents of my mail were of little to no concern to the trust.

I am very worried that just like Mid Staffs Hospital there is a possibility that boards and senior staff could be treating there own internal complaints with the same disregard that is shown to general members of the public.

I do not envisage any reply soon and I will update you all when I receive correspondence from the ICO

Dear Mrs Donnelly


It has been some time since we last spoke and I am fully aware that your post only concerns staff within the trust
being able to raise concerns.

My concern is that your trust wishes to pretend to be accountable and transparent however when issues are highlighted, specifically in relation to substandard care provision and record keeping your senior management including its CEO would wish matters to simply be ignored and dismissed.

My reason for writing to you today is that accountability and transparent conduct is imperative at all levels of the NHS and after my recent experience with your trust, Major issues raised with your CEO have not been addressed or made transparent.

These are Direct Threatening conduct towards a carer by a member of NHS Staff.

False and vexatious remarks by members of NHS Staff.

Fraudulent care records being created for and utilised by NHS Staff to discredit carers and to destroy family life.

I was of the belief that some of the above were criminal matters however to date no action has been taken by Staffordshire Police.

Over this next week I intend to publish a written article via the Internet and would be great full if you could provide me with your views.

Certain Staff members of your trust have not held back in there mistreatment of my family which I believe is nothing more than vexatious bullying.

I am firmly of the belief that these type of individuals deserve no employer protection and serve no use to A Trust which publicly says it is engaging in culture change.

This is not Transparency Stoke on Trent Partnership NHS Trust and neither is it Culture Change.

Well yet again I have not been keeping you good followers and readers updated on my fight to gain
accountability or any form of transparency out of the Stoke on Trent Partnership trust here in sunny Staffordshire.


The good news is that I have just returned from a short holiday and feel very relaxed and ready to continue in my endeavours.


So to update you all the recent threats of litigation have up to date not been forthcoming from the trust, This is because the trust and its CEO are fully aware of the damage that was caused to my family and the many lies and poor care that had been provided to us directly by there own staff.


In my last conversation with the trust they were still adamant that they would protect the reputation of there own staff.


It beggars belief that this trust still wish to protect deceit and lies and even now at this late date wish to try its best to avoid any accountability in its many failings.


As you folk may be aware the CEO of this trust promised me many months ago that he would provide me with a written report of the findings made by the trusts lead Social Worker.


The findings in part had been verbally reported by the CEO which included failure to carry out adult protection investigations and further reliance on possible fabricated documentation including no social care provision.


This report I believe shows the full extent of the Trusts and the County Councils Incompetence here in this part of Staffordshire.


Mr Poynor CEO has gone back on his promise to provide this report.


Unfortunately I have had to place a request under the Freedom of Information Act to gain access to this report.


The Trust initially refused the report under the grounds that as my granddad is dead they can withhold the document and that it would be to large to redact.


After my further contact and the intervention by the ICO the Trust have now revised there position to the following.


My Request is "Vexatious".


In my own view I should have the right to see in black and white the full extent of how this trusts staff have defamed and lied regarding my care for my late granddad.


I should further be able to have a copy of the report warts and all so that adult social care can not abuse or behave in this aggressive way towards my family in the future.


Staffordshire County Council tried to cover its failings and now the Trust and Mr Poynor can see the extent of this failure they now also wish all evidence withheld from public view.


This is not Transparency Mr Poynor and neither is it Culture Change.


I have informed the Information Commissioner of my views on the Trusts use of Vexatious as a attempt to withhold this very damaging report being made available.


The Trusts view is that I will use the report to Target individuals via twitter or Blogger.


For my use this report can be redacted (names removed) but the content needs to be in the public domain only then will I ever hold any hope of culture change in our NHS.


The report will be my insurance against any further attempt by this Trusts Social Workers to defame or tell blatant lies and failure to support hard working carers in the future in this area.


Hopefully the ICO will see my concerns as genuine.


In the meantime The Stoke on Trent Partnership Trust are due a visit by the Care Quality Commission, I have requested that even though the trust refuse to provide this maybe they should provide it along with the financial audit that showed financial irregularities in elderly peoples own monies to them so they can get a full view of how the elderly and there carers are treated.




As soon as I get the next update from the information commissioner I will let you know the outcome, The Fight for Justice continues.
